As you may already know, the practice has recently closed.  I will be joining Vanderbilt in Nashville.   It has been an honor and privilege to have been your health care provider and I will miss you all!

We will be around to answer calls for a few weeks. After this time, your health records from this Practice will be transferred to Morgan Records Management, LLC (the “Records Custodian”). The Records Custodian will continue to maintain your records in accordance with applicable confidentiality and security standards and with other applicable laws. Your records will be destroyed no less than 10 years after the last date of services you received from this Practice.
